Senior Independence Working to Find Solutions
The goal of Senior Independence is to extend and enhance the independence of older adults, and to offer support and education to those caring for them. Senior Independence’s Coordinet is a "care management" program that addresses the problems confronting older adults and their families. Coordinet is a one-to-one consultation service designed to help seniors and their families explore available options and make the right choices.
Here's how it works...
- A professional social worker experienced in older adult care will meet with you to conduct an in-home assessment.
- Based on the assessment, recommendations are shared with you about services, options and available resources.
- Together, a plan of care is developed and implemented.
- Ongoing assistance is available through periodic visits to assure that the plan is working smoothly.
- As needs change, the social worker will work with you to identify alternatives.
