Established in 1992, Swan Creek Retirement
Village is Northwest Ohio’s premier continuing
care retirement community.
Our beautiful 34-acre site includes protected woodlands, walking trails, and
of course, the charming Swan Creek.
Our continuing care retirement community offers villa
homes, independent living apartments, assisted living
apartments, long-term nursing care, short-term rehabilitative
care, and specialized Alzheimer's/dementia care. Home
health services and an adult day program are available
through Senior Independence.
