Personalized attention, a secure home-like environment and specially designed programs provide individuals with early memory loss/dementia with a sense of community and belonging in Swan Creek’s special care unit. Daily programming includes “failure-free” activities promoting a sense of accomplishment for our residents.

All suites are private and adjoin a comfortable, relaxing living room. A secured courtyard allows outdoor access. Residents are encouraged to bring personal furnishings. Special programs and activities are available and are tailored to the individual's strengths and needs with positive reinforcement to encourage residents to use their existing abilities.
