The goals of our Adult Day Services Center provided through Senior Independence focus on enabling participants to remain at home for as long as possible; providing respite and support for caregivers; easing transition from a hospital discharge to the home environment; and reducing the chance of premature placement in a nursing facility. Services are available Monday through Friday from 7 a.m. to 6 p.m.
Services and amenities include:
- Personal care
- Nursing care and assessment
- Nutritional meals and snacks
- Social services and counseling
- Therapeutic activities
- Rehabilitation therapy
- Emergency care and health-related services
- Transportation
To participate in the adult day service program, an individual must:
- Be 18 years of age or older
- Need intermittent supervision
- Have a physical and/or a mental impairment
- Be able to walk or transfer from a wheelchair with the assistance of one person
- Have a primary care physician
- Have an identifiable caregiver
- Be non-aggressive or non-abusive toward others
