Does paying off the mortgage with IRA assets trigger taxes and penalties?

Yes, in almost every case. A distribution from a traditional IRA before age 59 1/2 is subject to ordinary income tax on the full amount. It also triggers a 10 percent additional tax under IRC Section 72(t), unless one of the statutory exceptions applies.
